THIS NOTICE DESCRIBES HOW HEALTH, MEDICAL AND OTHER PERSONAL INFORMATION ABOUT YOU MAY BE COLLECTED, USED AND DISCLOSED BY CAREBOOK. PLEASE REVIEW IT CAREFULLY.

Carebook Technologies Inc.(“Carebook”) understands the importance of the privacy and security of your personal information. Carebook utilizes the personal information (including health information) of its clients in order to provide the services outlined within our Terms of Service agreement including preventive reminders, recommendations, notifications about health measures that may require attention, assessment of health risks and other functions. By using any Carebook service, you consent to the collection, use, and disclosure of your personal information in accordance with this notice and the applicable Terms of Service. How Carebook uses or discloses your personal information:

In order to provide services to you, Carebook is required to both collect and use your personal information (including health information).

Depending on the Carebook product you use, this information may come into your Carebook via authorized transfers from your electronic health record and labs, health questionnaires that you complete, information that you entered into your Carebook manually and/or as otherwise contemplated in the applicable Terms of Services.
Carebook only discloses your personal information: (i) to healthcare providers or other users that you have explicitly authorized and consented to within the Carebook application; (ii) to Carebook’s third party service providers who perform services on our behalf (such as our data hosting provider); (iii) to other third parties identified in the applicable Terms of Service; and (iv) to the extent required by law.

Carebook will only use your personal information for the purposes of providing you the applicable Carebook services and for any other purposes identified in the applicable Terms of Service. Please note that Carebook may take personal information collected through the Carebook application and anonymize it (“Anonymized Data”). Anonymized Data is not personal information and does not under any circumstances identify you. Carebook’s use and disclosure of Anonymized Data is not subject to the terms of this privacy notice and Carebook may use and disclose Anonymized Data for a number of different purposes including, without limitation, in connection with research in prevention, chronic disease management, and other wellness initiatives and to design wellness and health management programs.

One of the services of Carebook is the ability to receive notifications. These communications may include, but are not limited to information on new wellness initiatives, research, medical devices, medications and other products or services related to your Carebook profile. You may opt out of receiving such notifications at any time. If you opt in to receive these communications, the content will be exclusively delivered within your Carebook.

Your rights as a client of Carebook:

As a client of Carebook, you have rights to request a copy of your information as it exists within the Carebook system as well as request any corrections to your information. Should you make a request of this nature, Carebook is committed to responding within a 30-day period. Where information came to Carebook from another source (such as the electronic medical record or connected devices etc.), all data corrections may need to be performed in the source application. Carebook will help you to identify the source of information, but cannot represent you in communications with the third party. In the unlikely scenario that Carebook is unable to identify the source of your information you will receive a confirmation on this within 60 days in written format.

If at any time you as a client of Carebook wish to discontinue use of the Carebook system, please contact datarequest@carebook.com and we will ensure the removal of your account within a 30-day period. After removal of your account has occurred, your information may be retained by the Carebook system for a period of time as required by law. After the required period of time has passed, Carebook will securely dispose of your personal information.

Protection of your information:

Carebook places a high importance on protecting your information from unauthorized use or disclosure. Carebook employs a range of safeguards designed to protect personal information against loss and theft as well as unauthorized access, disclosure, copying, use, and modification, including physical, organization and technological measures, commensurate with the sensitivity of your personal information.

Where we use service providers who might have access to your personal information, we select them carefully and require them to have privacy and security standards that are comparable to ours. We use contracts and other measures with our service providers to maintain the confidentiality and security of your personal information and to prevent such information from being used for any other purpose.

Changes to this Notice of Privacy Practices:

When Carebook changes the terms of this notice which affects the way we treat your information, we will inform you via e-mail and also update the text of this notice on the Carebook web site.

Complaints:

If you have any questions related to our practices with respect to the collection, use or disclosure of personal information, including our use of service providers located outside of Canada, or if you wish to issue a privacy complaint to Carebook, please contact our privacy officer at the e-mail address provided in the header of this notice.

At no time will Carebook retaliate against you for filing a complaint.
